Applicant's Acknowledgment, Authorization & Release

I certify that all of the information provided by me in this application (and/or any other accompanying or required documents) is correct, accurate, and complete to the best of my knowledge and is without omissions, misrepresentations, or falsifications of any kind. I understand that the falsification, misrepresentation or omission of any facts in said documents will be cause for denial of employment or immediate termination of employment regardless of the timing or circumstances of discovery.

I understand that submission of an application does not guarantee employment. I further understand that, should an offer of employment be extended by IMS and accepted by me, that such employment with IMS or any of its affiliates is at will, for no specific duration, and may be terminated by either IMS or myself at any time, with or without cause or notice. I understand that none of the documents, policies, procedures, actions, statements of IMS or its representatives used during the employment process or during employment is deemed a contract of employment real or implied. I understand that no representative of IMS or it affiliates except the Corporate President or Chief Executive Officer has the authority to enter into any agreement guaranteeing any conditions of employment or any agreement contrary to the foregoing statements and that any such agreements must be approved in writing by the Corporate President or Chief Executive Officer.

I understand that if offered employment with IMS, I may be required to submit to a pre-employment medical examination, job-related skills and job knowledge tests, drug/alcohol screening, and/or a background check as a condition of my employment. I understand that unsatisfactory result from, refusal to cooperate with, or any attempt to affect the results of these pre-employment tests and checks will result in withdrawal of any offer or termination of employment if already employed.

I hereby authorize any and all schools, former employers, references, courts and any others who have information about me to provide such information to IMS and/or its representatives, agents, vendors and I release all parties involved from any and all liability for any and all damage that may result from providing such information.

If employed, I agree to conform to the rules, regulations, policies and procedures of IMS at all times and understand that such compliance is a condition of employment. I further understand that IMS has adopted testing practices to identify employees who use illegal drugs on or off the job or who abuse alcohol or prescription drugs on the job and that it is a condition of my employment with IMS to submit to such testing when requested.

I understand that an unsatisfactory result from, refusal to cooperate with, or any attempt to affect the results of such tests, during employment may result in termination of employment.

This application for employment is good for 30 days only.

Consideration for employment after 30 days requires a new application.
